Frankly speaking I never tried to append images. Moreover, I even do not try TIFFOpen() directly sonce I needed TIFFFdOpen() instead, but there is good chance that it will work for you. If it fails you have choice either modify TIFFOpen() or use the following sequnce instead
fd = _open(xxx, _O_BINARY|_O_RDWR);
tif = TIFFFdOpen(fd, xxx, "a");

Ok. Right now.

Generally speaking it is problem in Win32 - it has two (at least) function sets to deal with file descriptors: OpenFile()/ReadFile() etc. and _open()/_read() etc. You could imagine: file discriptor returned by _open() is not readable by ReadFile()! Since I have to use _open() I just add several defines to tif_unix.c and compile it under Windows. As far as I know there is no difference in functionality between _open() function set and UNIX functions (in binary mode) so it might help in other cases.
Below is tif_unix.c with my changes. Good luck!
