If someone could test this on windows, I'd happily check it in.

I applied this fix to CVS trunk and built libtiff on Windows using Microsoft Visual C++ 2010 Express (as reported in the original message) and the fix works. Pages of the PDF are now generated in order. Attached are the test tiff file and the generated pdf.
