That is all consistent, and all other stuff seems to be consistent too...

All your IFDs seem to have these properties. That means, I see nothing wrong with it. I'm not a LibTiff expert, but I'm pretty sure there's no reason why LibTiff would not be able to handle this type of image.

Either this QFaxReader is to blame, or perhaps there's something odd like a consistent bad compression or something. Have you checked with other apps that likely use LibTiff?
