FWIW, I've never seen one.
I expect that you will be able to see one today (CPU is crunching to generate the huge colormap). In the mean time, I am interested to know who is able to load and view the *beautiful* 4096 color (12-bits per sample) colormapped TIFF file I have made available at:
ftp://ftp.graphicsmagick.org/pub/outgoing/colormapped-12-bit.tiff
The current GraphicsMagick 1.2 development snapshot is able to read and write this file just fine.
MATLAB can read and display it. On export, though, MATLAB would write it using 16 bits per sample.
